CROSS-SHIPMENT AGREEMENT:

I am requesting Dynapro to cross-ship the product(s) above to replace defective product(s)  under warranty. By signing this form I understand that the product(s) authorized for return must be delivered to Dynapro within 10 working days from the date of receipt of the cross-shipment. Failure to deliver the product(s) will result in my liability for the value of the product(s). 

 

In order to process the cross-shipment request,  Dynapro needs a valid credit card and a signature. The credit card below will only be charged if Customer fails to abide by the cross-shipment agreement.

All products are shipped via FedEx Standard 2 Day (USA) unless otherwise specified. Customer must pay for Priority overnight shipping or provide their FedEx Account Number below.

  • This form will be returned to you by fax or email with an RMA number. If you do not get a response in 24 hours, please download the form and fax it to 732-780-3784. Products shipped to Dynapro without an RMA number will be refused.

  • The RMA number must be clearly marked on the outside of the package.

  • Damage or loss of goods during shipment is the sole responsibility of the customer.

  • Insufficient packaging may result in loss of warranty. RMA number is valid for 30 days.

  • Products returned for credit are subject to a 20% restocking fee.

  • No credits will be issued after 30 days of purchase.

  • Include a copy of RMA form with RMA number on it.

  • Include all necessary documents and drivers.

  • All warranties will be void if inspection finds that the product has been abused or altered without authority.

  • Product returned with expired warranties are subject to standard repair prices set forth by Dynapro.

  • Dynapro will not be responsible for system repair unless all original system parts remain in the system.

  • Dynapro will not warrant or replace any system that is not 100% Dynapro product.

  • Warranty void if Serial Number labels are removed.

Product Return Procedures:

 

Follow these procedures to ensure prompt and proper handling if a Dynapro product becomes defective during the warranty period.

1.  Call our Customer Service Department to obtain an RMA number. Please have the following information available when you call; Dynapro item number, packing slip with which the product as delivered, and reason for return.

2.  Include a copy of the packing slip “Proof of Purchase,” which includes the serial number list of the items being returned, with the returned merchandise.

3.  Attach a note describing the suspected defect with your return.

4.  Clearly mark the RMA number on the box.

5.  Prepay shipping charges.

6.  Return all products in their original packaging in which they were received from Dynapro. Improper packaging may void the warranty. Returned items that fail to include all the above information may be subject to a service charge. RMA number is valid for 30 days.

 

Return for Credit:

Dynapro will not issue credit for any item that can be replaced within 30 days. A minimum restocking fee of 20% will be charged against any returned merchandise received within one month of its shipping date. All merchandise must be in resalable condition.

 

Dead on Arrival (D.O.A) Products:

Dynapro must be called for an RMA number within 7 days of delivery. Returned product must be received by Dynapro within 10 days of the RMA number issuance date.
